Have these to hand and the referral form can be filled out in a few minutes.
- Participant details and consent to refer
- Plan management type — plan-managed or self-managed (we aren’t NDIS-registered, so NDIA-managed plans can’t be invoiced through us directly)
- Chosen device: Home Alarm, Mobile Alarm Wrist, or Mobile Alarm Pendant
- Monitoring preference: 24/7 Professional or Family Response

Umps isn't a registered NDIS provider, so participants on NDIA-managed plans can't be invoiced through us directly. Plan-managed and self-managed participants can both access the full range without restriction.
Common questions
Plan-managed: we invoice the plan manager. Self-managed: the participant (or their nominee) pays and claims back through the portal.
